You will then be redirected to complete the medical certificate information form. Please note:

• The form will not not save as you go along. Therefore, you must complete it in one go.

• You must provide accurate, current and complete information about yourself.

• You will need:

  • Your GP's contact details

  • Details on your current and previous health conditions and medications

  • Your height and weight

  • If you require a specific certificate or document to be completed, you'll need to upload an electronic copy of it 

As long as you are considered suitable, the doctor will provide you with a signed medical certificate or event form on the basis of no medical contraindication to your participation in the event.
